چکیده

Diffusion-weighted imaging(DWI)has revealed the reversal ofoften sizeable damage due to brainstem lesions in patients treated within 4.5 hours, and this reversibility of damage was strongly associated with early neurological improvement.1 In posterior circulation stroke, the reversibility of damage due to aDWI-detected lesion ispoorlydocumentedafter intravenous thrombolysis2 or intraarterial therapy.3,4 We report a case of vertebrobasilar stroke mechanically recanalized 4.5 hours after symptom onset that revealed substantial reversal of early ischemic damage to the brainstem on DWI scans. A51-year-oldman suddenlywent into comaafter experiencing bilateral flaccidity in the lower extremities. A noncontrast computed tomographic scan showed that there were no signs of cerebral ischemia. Magnetic resonance imaging of the brain was performed 3 hours after onset, and DWI scans showed right lateral pontine (Figure, A) and central pontine infarction (Figure, B) secondary to acute basilar artery occlusion (Figure, C). A mechanical thrombectomy using a Solitaire Flow Restoration (FR) stent (Covidien)was performedwhile the patientwas under general anesthesia, andcomplete recanalizationwasobtained4.5hoursafter stroke onset (Figure, D). The patientwas extubated 12 hours later, and recoverywas complete on day 1.Magnetic resonance imaging onday 4 revealed total reversibility of the damage to the right lateral portion of the pons due to the lesion (Figure, E) and significant reduction inthevolumeofthecentralportionof thepons(Figure,G).These findingswere also observed in fluid-attenuated inversion recovery sequences (Figure, F-H).

Angioscopic observations after percutaneous thrombectomy of thrombosed hemodialysis grafts.

PURPOSE To use angioscopy to evaluate and compare the amount of residual thrombus and endoluminal wall damage in hemodialysis grafts after percutaneous thrombectomy procedures. MATERIALS AND METHODS Thirty-nine thrombectomy and angioscopy procedures were performed in 35 patients. Percutaneous thrombectomy methods included eight different mechanical thrombectomy devices and the "lyse and wait"...

New Cerebral Microbleeds After Mechanical Thrombectomy for Large-Vessel Occlusion Strokes

The interval appearance of cerebral microbleeds (CMBs) after endovascular treatment has never been described. We investigated the frequency and predictors of new CMBs that developed shortly after mechanical thrombectomy for acute ischemic stroke, and its impact on clinical outcome. We retrospectively analyzed patients with large-vessel occlusion strokes treated with Merci Retriever, Penumbra Sys...

Immunohistochemical analysis of thrombi retrieved during treatment of acute ischemic stroke: does stent-retriever cause intimal damage?

BACKGROUND AND PURPOSE To provide evidence whether mechanical thrombectomy with stent-retrievers in the treatment of acute ischemic stroke causes intimal damage. METHODS This study analyzed thrombi retrieved by mechanical thrombectomy from cerebral arteries in 48 consecutive patients with acute ischemic stroke for the presence of endothelial cells using CD34 antibodies. RESULTS Of 48 thromb...

Acute basilar thrombosis: Recanalization following intravenous thrombolysis is dependent on thrombus length

INTRODUCTION We investigated whether thrombus length measured in Computed Tomography Angiography (CTA) is predictive of the success rate of intravenous thrombolysis (IVT) in acute basilar occlusion and whether recanalization can be achieved by additional mechanical endovascular thrombectomy. METHODS In 51 patients with acute basilar thrombosis thrombus length was measured on CTA images before...

Adjuvant Tirofiban Injection Through Deployed Solitaire Stent As a Rescue Technique After failed Mechanical Thrombectomy in Acute Stroke

PURPOSE We present our experiences of intra-arterial tirofiban injection through a deployed Solitaire stent as a rescue therapy after failed mechanical thrombectomy in patients with acute ischemic stroke. MATERIALS AND METHODS Data on 18 patients treated with adjunctive tirofiban injection through a temporarily deployed Solitaire stent after failed mechanical thrombectomy were retrospectively...
